First step: View the group descriptions. Discuss and get confirmation from Coach Amy on group placement. 

Complete Registration Packet : 

• Recurring Credit Card Authorization Form (linked below)
• Safe Sport Agreement Form (linked below)
• Covid-19 Liability Release Form (linked below)
• Pay registration and first month dues 

Send completed forms listed above electronically to [email protected] and send payment of registration and first month dues by one of the following  methods:

All subsequent monthly dues will need to be paid via one of these methods by the 1^st of each month or the credit card on file will be charged with all associated bank fees.
